Welcome to Wellness Elevated

A heart health education brand powered by Uniquely Defined, LLC.

We bring awareness, empathy, and action together to help adults recognize early signs of congenital heart defects, advocate for appropriate screenings, and take ownership of their wellness journey. We believe everyone has the power to define who they are destined to become—especially when it comes to their health.

At Wellness Elevated, our mission is to help adults recognize early signs of congenital heart defects, advocate for screenings, and take ownership of their wellness journey. Through education, community outreach, and actionable tools, we are shifting the conversation around adult heart health—one heartbeat at a time.

Rhythm of the Heart™ is a research-informed curriculum designed to help communities recognize symptoms of congenital heart defects earlier and strengthen heart health education nationwide.

Expert-Led. Community-Driven. Purpose-Built.

Wellness Elevated is founded and led by Dr. Jacklyn Sanders, PhD—Community Nutritionist, Extension Specialist, and heart health advocate with over 17 years of experience in public health education and community engagement.

Evidence-Informed Heart Health Education

Rhythm of the Heart™ was developed from doctoral research examining the lived experiences of adults who were diagnosed with congenital heart defects later in life.

This research revealed a common pattern: many individuals experienced symptoms for years before receiving an accurate diagnosis. Symptoms such as fatigue, shortness of breath, dizziness, and reduced stamina were often normalized or misattributed to other conditions.

These findings highlighted a critical need for community-based education that helps individuals recognize potential warning signs earlier and advocate for appropriate medical evaluation.

The Rhythm of the Heart™ curriculum translates these insights into accessible educational programming designed to:

• increase heart health awareness
• strengthen community health literacy
• encourage early medical consultation when symptoms persist

By bridging research and community education, Rhythm of the Heart™ helps empower individuals to better understand their heart health and make informed decisions about their care.

Developed from doctoral research on adult congenital heart defect diagnosis and community health education.

See Rhythm of the Heart in Action

Rhythm of the Heart was developed to bring heart health awareness directly into communities. Through workshops, health fairs, and educational sessions, individuals learn how to recognize symptoms, advocate for their health, and take steps toward prevention.

Today, community educators and health professionals can also become certified facilitators and bring this program to their own organizations and communities.

Bring This Education to Your Community

Health educators, Extension agents, and community leaders can become certified Rhythm of the Heart facilitators and deliver this program in their communities.
