About Uniquely Defined
Empowering Adults to Take Control of Their Heart Health
At Uniquely Defined, we believe heart health is more than numbers — it’s about empowerment, awareness, and advocacy.
Meet Dr. Jacklyn “Jaki” Sanders
Hi, I’m Dr. Jacklyn Sanders—most people call me Jaki—and I believe your heart has a rhythm worth listening to.
For over 17 years, I’ve worked in community nutrition and health education. But my turning point didn’t happen in a classroom—it happened in my own body. I was diagnosed with a congenital heart defect as an adult, after years of symptoms that were overlooked or dismissed. That experience was both life-changing and mission-defining.
It’s why I founded Uniquely Defined and developed the Rhythm of the Heart™ curriculum—to close the gaps I once faced and to help others get the answers, resources, and support they deserve.
I’m here to guide, educate, and walk alongside you on your wellness journey—whether you’re experiencing symptoms, advocating for someone you love, or simply wanting to learn more.
Let’s define your wellness—together.

From Research to Real-World Impact
My Ph.D. in Health Education and Promotion focused on the lived experiences of adults diagnosed with CHDs later in life. From that work came Rhythm of the Heart™—a curriculum that blends evidence-based learning with personal reflection to help communities recognize the signs of heart conditions and take proactive steps toward wellness.
Community workshops and events that turn awareness into action
One-on-one wellness consultations and advocacy coaching
Digital resources that make heart health education accessible
The Heart Behind the Name
Uniquely Defined grew from my passion for poetry and purpose. The name reflects my belief that you have the power to define who you are destined to become. This work is deeply personal—and it’s my promise to show up authentically, share boldly, and serve with heart.
“You have the power to define who you are destined to become.”
